'Failed to load sentinel' on death #1178

Describe the bug
When I die, the screen goes white and never does anything else. The console says:

[DBG]: Failed to load sentinel
FAILED

many, many, many times and overflows the terminal line limit.

To Reproduce
Anytime I die, this happens.

Expected behavior
Waking up at the hospital.
